The Prowers County Sheriff's Office would like to provide forms for your use. These forms are the most commonly used during investigations or during other incidents used by our office. 

**Please Report any crimes immediately. Failing to report criminal episodes immediately can degrade evidence at the scene and prevent or seriously hinder any investigation that might be necessary.** 

These forms are for incidents where you have access to a computer and you may have lost the form(s) given to you. Or you may want to download and print some of the stolen item forms to place with your household inventory items, in case of loss of such object.
