Immunizations: Immunizations for infants, children, and adults are available by appointment at clinics held several times a month. If needed, individual appointments are available and services for walk-ins are provided when staff is available. School immunization clinics are also held during the year. Annual flu clinics are offered at several locations in the fall. Pneumonia vaccine is available at these clinics, as well as by appointment at the office throughout the year. There is a minimal fee charged for the administration of immunizations for children. No one will be denied services due to the inability to pay. Adult immunization costs vary, depending on the type of vaccine given. Prowers County's immunization rate is one of the highest in the state. BE WISE IMMUNIZE!

Communicable Disease Control:
√ Case Finding/tracking of all reportable diseases
HAN - Health Alert Network System (Local, State, and Nationwide)
Presumptive Eligibility:

Prowers County Public Health is a site for pregnant women applying for Medicaid. Applicants who have medical verified pregnancy and who meet income guidelines are presumed eligible for Medicaid and can receive immediate care for herself and her unborn child.

Child Health Plan Plus:

CHP+ provides health and dental care for children age 18 and under who are not eligible for Medicaid and not covered by other health insurance. Eligibility is based on income and family size. Applications and assistance in applying for CHP+ are available at our office.

Healthcare Program for Kids with Special Needs (HCP):

HCP provides assistance in making and coordinating appointments for specialty care, along with case management for children with special needs who meet state and local guidelines. In addition to referral to board certified specialists, clinics available in Prowers County are Hearing and Neurology. Referrals may be made to HCP Clinics outside the county.

Early and Periodic Screening Diagnosis and Treatment Program (EPSDT):

EPSDT is the health care benefits package for Medicaid-enrolled children through age 20. It helps families get early identification and treatment of medical, dental, vision, mental health and developmental problems for their children. Assistance in accessing these services, including location providers, is provided by an EPSDT Care Coordinator under the supervision of a Registered Nurse.

Options for Long Term Care:

The Home and Community Based Services for the Elderly, Blind and Disabled is an ALTERNATIVE TO NURSING HOME PLACEMENT that allows Medicaid eligible persons with functional impairments to receive a wide array of services in the community.

School Health Services:

Prowers County Public Health provides services to Holly, Wiley, and Granada Schools utilizing RN's through contractual agreement. Services include:

√ Special Education Services
√ Referral services and assistance

Nurse-Family Partnership:

Helping first-time parents succeed is the goal of the Nurse-Family Partnership Program. Specially trained Registered Nurses make home visits during the pregnancy and until your child is two years old. The nurses help prepare you for labor and delivery, assist you in making healthy choices for you and your baby, help you be a better parent, and help you plan for the future. The program is voluntary and the services provided are at no cost to qualified low-income families. The program offers services to parents in Baca, Bent, Kiowa, and Prowers counties. Anyone can refer clients to the program...just ask us!

Women, Infants, and Children

WIC is a program designed to provide information on nutrition and nutritional assistance to children from birth to age five, pregnant women, and breast-feeding mothers. This is a federally funded program that has been proven to significantly improve the health of participating families.
