Home and Community-Based Services are for those individuals who are Elderly, Blind, or Disabled and Medicaid-qualified. This program is alternative care that is provided in the client’s home to prevent or postpone nursing home placement.

Services that can be provided by HCBS providers are; unskilled personal care, light duty homemaker services, non-medical transportation through PATS bus, respite care, electronic monitoring, and alternative care facilities.

As case managers, we assess for functional needs to determine if and what HCBS services are appropriate and cost effective. We continue to monitor the client with a care plan throughout the year and complete a yearly re-assessment.

You, a family member, friend or interested person may make a referral to the program on your behalf at Prowers County Public Health, Options for Long Term Care, or the Options for Long Term Care in Springfield.
