(Program Eligibility and Application Kit)

Colorado PEAK (Program Eligibility and Application Kit) allows individuals to screen for potential eligibility and check the status of existing benefits for Food Stamps, Adult Financial, Colorado Works, and Medical Assistance Programs. If you do not have access to a computer Colorado PEAK has computer kiosks available at Prowers County Department of Social Services, Prowers County Public Health, and High Plains Community Health Centers for clients to use free of charge.
